Assessment Part A: Proposed assessment for formulation Further information to obtain in order to construct a sound and helpful formulation for Kelley is information to fill in each of the three levels of assessment that would be used in assessing Kelly; Level 1 (Descriptive), level 2 (Cross-sectional) and Level 3 (longitudinal) As for Level 1 (Descriptive), based on Padesky and Mooney’s five-system framework (Padesky & Mooney, 1990), her presenting problems would be broken into the four internal systems and one external system. Therefore we should aim to get information to fill in each system as follows; 1. Cognitions - words or images that go through Kelly’s mind when her problem is present ( e.g. I can’t always believe doctors, because they are not always right) 2. Emotions or affect - how she feels when her problem is present (e.g. tried, worry and unhappy) 3. Behaviour- her outward actions that are visible to others ( e.g. visiting doctors more often than before, asking for reassurance that her symptoms are not a sign of something more serious) 4. Physiological changes/body symptoms (what happens in her body, such as a racing heart beat, headaches, stomach aches and sweating) 5. Environment is an external system (eg. the reaction of others or the stresses of particular domestic circumstances)
